  • 科学大师的故事:生物学家巴斯德

    巴斯德在历史上被公认是对人类最有贡献的科学家。他发现微生物是造成人类疾病的主要原因;控制病菌,就可以治疗,甚至预防疾病。他在传染病与免疫学上的贡献,使世界上每一个角落的人都受到帮助。巴斯德(Louis Pasteur)生于一八二二年法国西部的多里(Dole)城,他的父亲是拿破仑大帝麾下英勇善战骑兵队的军官。但是战败之后的父亲,看着法兰西帝国从兴起到衰亡,有一个体会,他告诉巴斯德说:“对法国最危险的不是巴黎街头的不法分子,而是在知识界向社会散布错误理论的人。”于是巴斯德从小就决定,长大要成为一个大学教授,以知识界作为他的战场,像骑兵般的直捣黄龙。因为立志要做一个老师,他在求学期间就给自己严格的要求,就是对每一科目都力求完美,因为日后要成为专门学问的人,需要有广博的知识作为基础。他的中学老师给他的评语是“认真”。在十九岁时,法国最好的巴黎师范大学以第十九名录取这个来自偏僻小镇的学生,他却拒绝了,认为自己还不够好。经过另一年的努力,以第五名被录取,他才自觉满意的进去就读。大学里的教授都发现巴斯德这个年轻人与众不同。有位教授写着:“在这个时代,能看到巴斯德认真、热忱、不为名利的工作态度,是一个老教授教书生涯的最好报酬。”二十五岁时,巴斯德取得物理博士学位。有位大学教授看他那么踏实努力,在他毕业后不久,就把女儿萝兰(Marie Laurent)嫁给他,用功的人最后还是不吃亏。萝兰小姐嫁后不久,写信向父亲抱怨,她的丈夫常常一星期也不说一句话,一个人关在实验室里,好几个小时。她在门外偷听,里面安静得好像没有人存在,她不知道嫁给什么科学怪人。她的父亲回信道,“让他保持这样,日后他会成为第二个伽利略。”还是老岳父独具眼光。但是萝兰小姐并不满意这个答案,她要嫁给一个爱她的丈夫,而非第二个伽利略。不久她逐渐发现丈夫的秘密,在巴斯德孜孜不倦、努力研究的背后,有一个动机,就是为了探索生命的奥秘。原来当时的欧洲大陆在知识分子中流行的是“自然发生论(spontaneous generation)”,认为生命可以由没有生命的物质中自然产生。在东西方的古文明里都记载着,腐烂的木头可以生出蛆来,腐烂的肉可以长出苍蝇,甚至希腊的爱神亚法罗莱特(Aphrodite)是由海水的泡沫产生。当时的名哲学家赫尔姆(Helmont)宣称,只要在老鼠笼内撒些面包屑,笼子内就会迸出老鼠来。一八五九年,达尔文发表进化论的《物种起源》,更被自然发生论拥为经典,生命可以由物质产生。 这时候起来反对的就是巴斯德,他认为如果物质本身可以产生生命,那生命变成短暂的,物质反成永恒的。他认为生命才有永恒的价值,因为这是神所创造的,而物质只是短暂的。他在一八五九年至一八六一年,仔细地以准确的实验,将加温煮沸的肉,放在开口弯曲的瓶中——什么虫也长不出来——因而强而有力的驳斥了当时的理论。现今在生物学课本上都记载了巴斯德实验的正确性。但是当时几乎所有的科学家都反对他。然而巴斯德仍然坚持他的看法,并提出食物的腐烂是微生物的作用,他说:“微小的细菌,看起来是静止的,但是只要有合适的环境,也会遵守生命的法则来活动。”又说:“物理与化学是生命的现象,只有神才是生命法则的作者。”这一宣称使得反对他的人更为激烈,纷纷提出棘手的问题来刁难他。具有骑士精神的巴斯德,也举起科学长矛,奋勇作战。一八六七年,有人质问他:法国的蚕为什么会生病?他根据三年实验结果,分离出二种致病的杆菌,发现了治疗这种疾病的方法,并且无意间拯救了法国的蚕丝与服装业。又有反对他的人提出:酒为什么自然会变酸?一八七O年,他提出那是微生物的作用,并且提出高温杀菌法,使酒保持新鲜;同样的方法也可使牛奶保持新鲜。他又拯救了食品业,反对他的人只好勉强送他一个勋章。后来十二年期间,有人陆续问他羊的炭疽病、猪的红斑丹毒病、鸡瘟与被视为绝症的可怕狂犬病,他都一一找出病毒,并用方法解救,发展出免疫学与传染病控制学的先河。有趣的是,他的重要发现,都是源自他的对手提供的难题,而非自己去找来的。 巴斯德老年时,回到自己的母校演讲,说到他的一生在面对如此大的反对,而能节节得胜的原因有两个:“信心,相信神的启示…… 信心是一条绳子,维系你周遭所发生的事情,与你内心的呼召,成为一个和谐的关系。热心(enthusiam),这是最好的字,是En及Theo合成,En是里面,Theo是神。真正持久的热心是来自上帝住在我的心里。”一八九五年他在法国病逝,临终前说道:“太奇妙了!我一生的工作,太奇妙了!”

    [color=#404040][/color]继治疗白内障“神药”莎普爱思滴眼液被曝光调查之后,又一个“神药”匹多莫德被《问药师》创始人、北京和睦家医院药师门诊主任、中国协和医科大学药学硕士冀连梅扒皮。据她透露,这个进口药在国外医学临床试验尚处于小白鼠阶段,疗效尚不明确,但在我国却摇身一变成了价格昂贵的“神药”,在各大医院儿科滥用,销售额预计达到40亿元。冀连梅建议相关部门对匹多莫德的临床疗效进行再评价。[b]现状儿科“神药”几乎“是病就开”[/b]冀连梅发文章反映,在微信公众号里的网络咨询平台上,几乎每天都能看到关于匹多莫德口服液的咨询。粗粗浏览一遍,她才发现这个药是儿科、耳鼻喉科和皮肤科医生们的宠儿,从这三个科室看完病出来的孩子,几乎每人手里的药单上都有匹多莫德的身影,流水线一般:预防感冒来一盒、发烧咳嗽来一盒、鼻炎扁桃体炎来一盒、湿疹荨麻疹来一盒……每盒的单价从几十到上百元不等,而且一开就是一个月的量,一吃就是三个月的疗程,难怪医生们会对它推崇备至!冀药师在网上检索到的数据显示,匹多莫德在我国销量非常好,2016年匹多莫德在国内等级医院销售额达到了35亿,在零售药店的销售额是4.27亿,再加上在其他非等级医院的销售额,“我猜总额会达到40亿!而这其中,绝大多数由儿童患者买单。”她在文章中说。[color=#404040][/color]北京青年报记者在网上药店查询到,匹多莫德大致分为胶囊、颗粒剂、分散片、口服液四种剂型。其中颗粒剂、胶囊、分散片均为国产药,价格每盒三四十元,而口服液是意大利进口药,价格昂贵,单盒原价194元,促销售价140元。在该口服液的用法和用量上,百度百科上可以查询到,儿童发病期,每日两次,共两周或遵医嘱。如果是用于预防疾病,每日一次,至少60天或遵医嘱。百度百科上显示,该药为免疫刺激调节剂,适用于机体免疫功能低下的患者;上、下呼吸道反复感染(咽炎、气管炎、支气管炎等);耳鼻喉科反复感染(鼻炎、扁桃体炎、鼻窦炎、中耳炎);泌尿系统感染;妇科感染;并可用于预防急性感染,缩短病程,减少疾病的严重程度,可作为急性感染期的辅助用药。[b]质疑“神药”存临床疗效不明确等四大问题[/b]检索国家食药监局官网可以查询到,匹多莫德口服液是进口药,产自意大利,发证日期2015年12月2日,有效期截至2020年12月2日。但是冀连梅调查国外各大医学网站后发现,这个产自意大利的进口药在国外医学研究中显示,临床疗效和安全性均不明确。据冀连梅总结,该药存在四大问题。[color=#404040][/color]问题一:研究者主要集中在中国和极少数几个欧洲国家如意大利、俄罗斯。排在最前面的两篇新出版文章就是中国医生的文章,可惜还停留在临床前的动物研究阶段,不是仔鸡就是小白鼠,对临床没有实际指导意义。问题二:2015年,国外曾有一项针对57名3岁刚入幼儿园的健康孩子预防急性呼吸道疾病的研究。但是经过随机双盲安慰剂对照临床研究结果显示,与安慰剂相比,匹多莫德并没有显示出统计学意义上的预防急性呼吸道感染的作用。问题三:令冀连梅无比惊讶的是,就在国外权威循证医学库考科兰(Cochrane)里,都没有关于匹多莫德用于儿童安全有效的研究证实文献时,国内的研究结果却几乎清一色的有效!这些结果表明匹多莫德对反复呼吸道感染、哮喘、泌尿系统感染、妇科感染、丙型肝炎,甚至非感染性疾病如白癜风、肿瘤、过敏性紫癜等病均有较好的预防或治疗效果,简直成了万能的神药。问题四:学龄前儿童本身的免疫系统还在不断发育成熟中,不应该被轻易冠上免疫力低下的帽子而随意用药。除了疫苗可以预防疾病提高儿童免疫力外,欧美主流国家没有所谓提高儿童免疫力的药。[b]建议“神药”疗效应启动再评价[color=#404040][/color]冀连梅认为,匹多莫德在我国热销与儿科医生喜欢开这个药密切相关。她分析,医生之所以喜欢开,主要是基于以下三个因素。首先是创收。在外界看来,医院推行药品零差率后医生开贵药的动力会减少。不过,冀连梅认为,零差率只是减少了医院的加成,并没有斩断药代给医生的回扣。其次是照说明书机械开药。在冀连梅看来,有些医生不太关注真实疗效如何,有一部分医生从来不查原始文献,说明书怎么写他就怎么开药,他的老师怎么教他就怎么信,没准他自己家人朋友他也这样开、这样吃呢。最后是还存在医生为了安慰或者应付患儿家长的因素。[color=#404040][/color]冀连梅认为,既然这个药已在国内上市多年,已经有这么多患儿使用,因此她建议行业主管部门国家食药监总局启动对这个药的再评价,比如设计科学严谨的大样本量回顾性研究来评价这个药的真实疗效和安全性。据悉,2016年3月,此药在巴基斯坦申请上市时,该国监管部门就听取了专家组的意见,即:尽管这个药在中国、韩国、俄罗斯等少数几个国家上市多年,但它没有被收录到任何一本标准的药理学教材,也没有被欧盟药品监管部门和美国药品监管部门批准上市,它的有效性和安全性仍需要进一步评估,在有可靠的研究证据出现之前,不推荐上市使用。[/b]
